These Terms & Conditions ("Terms") are a legal agreement between you and GWA Ventures, LLC, doing business as Axiome ("Axiome," "we," "us," or "our"). They govern your use of our website and services.
By using our website, creating an account, or placing an order, you agree to these Terms and to our Privacy Policy and Refund Policy. If you do not agree, DO NOT use our services.
Axiome is not a pharmacy, a medical practice, or a healthcare provider. We do not sell, prescribe, dispense, or distribute prescription medication, and we do not practice medicine or provide medical advice.
Axiome operates a website that lets you request access to telehealth consultations and, where a licensed provider determines it is clinically appropriate, prescription products fulfilled by licensed third parties. Our role is limited to marketing, account administration, customer support, and coordinating the non-clinical parts of that process.
All medical evaluations, clinical judgments, and prescribing decisions are made exclusively by licensed healthcare providers who exercise independent professional judgment. Axiome does not employ these providers, does not direct or influence their clinical decisions, and has no authority to prescribe. Nothing on this website is medical advice.

Our services are available only in the United States, and only in the states listed below. We do not currently serve Texas, Mississippi, New Jersey, or Alaska.

A provider may also decline to prescribe, request more information, recommend a different treatment, or refer you to in-person care.
Placing an order does not guarantee that you will receive medication. You are purchasing access to a clinical evaluation — not a specific medication or outcome.
Prescriptions are issued only after review by a licensed healthcare provider, and only when that provider determines, in their independent professional judgment, that treatment is clinically appropriate for you. If the provider determines it is not appropriate, no prescription will be issued and no medication will be dispensed.
Results vary. We do not guarantee any specific result, outcome, or level of improvement. Individual responses to treatment differ, and treatment outcomes depend on factors outside our control, including adherence, medical history, diet, and activity.
Telehealth consultations are provided by Beluga Health, P.A., an independent Florida professional corporation, through its network of licensed healthcare providers. Your provider-patient relationship is with Beluga Health and its providers — not with Axiome.
The technology platform, electronic medical record, patient portal, and payment processing are operated by FUSE Health Inc. Prescriptions are dispensed and shipped by licensed pharmacies. The current dispensing pharmacy is The Pharmacy Hub, which works with Beluga Health under its partnership with FUSE. The dispensing pharmacy may change; the pharmacy filling any given prescription will be identified in your order records.
These parties are independent of one another. Axiome does not control the clinical services provided by Beluga Health or the dispensing services provided by the pharmacies, and is not responsible for their acts or omissions.
Complaints about clinical care, prescriptions, or medication safety are handled by the clinical partner. If you raise such a concern with us, we will transmit it promptly to the appropriate clinical contact.
Telehealth involves the use of electronic communications to enable a provider to evaluate and treat you remotely, without an in-person examination. You will be asked to review and agree to a separate informed consent to telehealth before your consultation.

FUSE Health Inc. serves as the Merchant of Record for patient transactions. Card payments are processed by Stripe. The descriptor on your statement may therefore differ from "Axiome." [[CONFIRM statement descriptor with FUSE]]
By providing a payment method, you authorize charges for the amounts disclosed to you at checkout, and you represent that you are authorized to use that payment method.
When you are charged.
You complete your medical intake, select a program, and submit your request for provider review. Your payment is processed only after a licensed provider approves your prescription and it proceeds to the pharmacy for fulfillment. That approval is the point at which payment is taken.
If the provider does not approve a prescription, you are not charged.
Prices are shown in U.S. dollars and may change. Any applicable taxes and shipping charges will be disclosed before you complete your order.
Some programs are offered on a recurring basis. If you enroll in a recurring program, you authorize us to charge your payment method on the schedule disclosed at checkout until you cancel.

Continued refills are subject to ongoing clinical review. A provider reviews your protocol periodically and may modify or discontinue it.
Refunds are governed by our Refund Policy, which is incorporated into these Terms.
Axiome does not accept returns. We do not handle prescriptions or medication, and nothing is shipped to you by Axiome, so nothing can be returned to us. Separately, prescription medication cannot be returned, restocked, or resold once it has been dispensed, for safety reasons and as required by pharmacy law. DO NOT mail medication back to us.
Axiome does not issue refunds or replacements. FUSE Health Inc. is the Merchant of Record and determines and issues any refund; the dispensing pharmacy is responsible for fulfillment. Contact us about any billing or order problem and we will bring it to the responsible party and follow it through, but the decision is not ours to make.
Our services are not for medical emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room. If you are having thoughts of suicide or self-harm, call or text 988 to reach the Suicide & Crisis Lifeline.

Providing false information can result in an inappropriate prescription and can seriously harm you. We may suspend or terminate your account for any violation of these Terms.
The Axiome name, logo, website design, text, graphics, and other content are owned by GWA Ventures, LLC or its licensors and are protected by intellectual property laws. We grant you a limited, revocable, non-exclusive, non-transferable license to access the website for your personal, non-commercial use. You may not copy, reproduce, modify, distribute, or create derivative works from our content without our written permission.
Our website and services are provided "as is" and "as available," without warranties of any kind, express or implied, including implied warranties of merchantability, fitness for a particular purpose, and non-infringement. We do not warrant that the website will be uninterrupted, secure, or error-free.
Content on our website is for general informational purposes and is not a substitute for professional medical advice, diagnosis, or treatment. Always seek the advice of a qualified health provider with any questions about a medical condition. Never disregard or delay seeking medical advice because of something you read on our website.
Axiome makes no warranty regarding the clinical services provided by Beluga Health or the dispensing services provided by any pharmacy.
To the fullest extent permitted by law, Axiome and its officers, members, employees, and agents will not be liable for any indirect, incidental, special, consequential, exemplary, or punitive damages, or for lost profits, data, or goodwill, arising out of or relating to your use of our website or services.
To the fullest extent permitted by law, our total aggregate liability arising out of or relating to these Terms or our services will not exceed the amount you paid to us in the [[TERM — e.g. twelve (12) months]] preceding the event giving rise to the claim.
Some states do not allow the exclusion or limitation of certain damages, so some of the above may not apply to you. Nothing in these Terms limits any liability that cannot lawfully be limited, including liability for personal injury caused by negligence where such limitation is prohibited.
You agree to indemnify and hold harmless Axiome and its officers, members, employees, and agents from any claims, damages, losses, liabilities, and expenses (including reasonable attorneys' fees) arising out of your misuse of our services, your violation of these Terms, your violation of any law, or your provision of false or incomplete medical information.
